2. Bring Form
Bring in the Oral Health Assessment Form to your appointment.
3. Get Copy
If you did not receive the Oral Health Assessment Form from your child's school, you may ask our front desk to provide you with a copy.

4. Form Signing
Once the check-up is complete, a dental professional will sign your child's form which will then be ready to be submitted to their school.
5. Recent Exams
If your child recently had a full dental exam, the appointment may not be required; you may simply bring the form to your home office to be signed by the dental professional.
